Sierra Leone Becomes First in Region to Formally Adopt Clinical Officer Title

The International Academy of Physician Associate Educators (IAPAE) is pleased to acknowledge a significant professional milestone in West Africa.

Sierra Leone has officially become the first country in West Africa to formally adopt the title Clinical Officer, with graduates now receiving permanent practice licenses from the Medical and Dental Council of Sierra Leone. Clinical officers are comparable to physician associates/assistants (PA) found in 63 countries and 5 US territories. The PA title has been in use in Seirra Leone since 1966 when the country first adopted the PA concept following the introduction of the health profession in the United States.

This development in Sierra Leone marks an important step in strengthening professional identity, regulatory clarity, and career progression within the region’s clinical practitioner healthcare workforce as more than 15 countries in Africa use the term clinical officer.

Professional Harmonization and Regulatory Recognition

Under the newly formalized structure degree holders with a 4-year Bachelor of Science in Clinical Medicine are recognized as clinical officers. Diploma holders with a 3-year diploma in clinical medicine will continue to be recognized as physician assistants.

This structured differentiation enhances transparency in qualifications, aligns training pathways with regulatory recognition, and supports clearer professional advancement routes. Formal licensure through the national regulatory body represents a critical advancement in ensuring accountability, professional standards, and system integration.

Strengthening Professional Identity in the Region

The harmonization of titles and recognition pathways contributes to greater clarity in workforce roles, stronger regulatory oversight, improved professional mobility and alignment with evolving international workforce frameworks.

As countries across Africa and globally continue to refine models for clinical practitioners, Sierra Leone’s step demonstrates the value of structured regulation and professional standardization.
